Healthcare district's annual grant application process begins

FALLBROOK – Each year since 1999, a portion of the Fallbrook Healthcare District’s revenues (derived from local property taxes) have been directed back into the community through grants that fund and sponsor programs and services that impact on the health and well-being of the residents in the district.

There are seven phases in the grant program process, beginning with submission of a Request for Proposal.

The Request for Proposal (RFP) replaces Fallbrook Healthcare District’s (FHD) previous Letter of Intent. The RFP should be a brief, but concise, description of the proposed program, its target population, its cost and support of its own organization’s mission. The program proposed must be health related (treatment, education, prevention) and must be delivered by non-profit agencies serving residents of the Fallbrook, Bonsall, Rainbow and DeLuz area.

Packets defining the RFP requirements as well as the entire grant process and its phases, are available to interested, qualified agencies by contacting the district administrative offices by e-mail, at [email protected] or by calling (760) 731-9187. All qualified agencies that secure a packet and submit an RFP will be considered.

RFP’s will be reviewed by the directors of the board, with acceptance or decline determined in the board meeting on April 14, 2010.

If the RFP is accepted, an invitation to present a full grant application will be extended by the board. If the RFP is declined, the agency will be notified and no invitation to apply will be extended.

The full grant application will require a more detailed presentation of the proposed program inclusive of project budget, project narrative and financial statements of the organization with a two year comparative balance sheet.

Completed grant applications are due in the district administrative office by Monday, May 3, 2010.

Grant Year 2010- 2011 runs July 1, 2010 through June 30, 2011. Grant awards will be presented in the Board meeting of July 2010.
