Fallbrook Home Health and Hospice expands to new location

FALLBROOK – Fallbrook Home Health and Hospice has announced that it has expanded and relocated to 521 E. Elder Street, Suite #208.

“As an affiliate of Fallbrook Hospital, we proudly identify ourselves both with the hospital and the greater community in which we serve,” said Pat Smorzewski, RN, Agency Administrator. “We take this opportunity with our new and larger headquarters to renew our commitment to our patients, their families and our partners in the healthcare community.
