Care advocate hired by Fallbrook HealthCare Foundation

FALLBROOK — The Fallbrook HealthCare Foundation is pleased to announce their newest service to the community: the Care Advocate program.

The program is designed for seniors and people with chronic health problems who may live alone, feel isolated, be unfamiliar with local resources or who are simply struggling to meet their needs on their own.

The Foundation has hired Ann Armenta to be the care advocate. Ann has her Master’s degree in social work from Washington University in St. Louis. Her professional experience includes working with community and health education programs and coordinating mental health services. She was most recently employed by the County of Los Angeles.

As the care advocate, Ann will visit seniors in the privacy of their own homes to get to know their unique situation and needs. She will then provide the senior with referrals to resources and, if needed, assist the senior in accessing services. Ann will follow up with the senior on a regular basis for as long as needed to ensure that the resources are effective and to meet any additional needs that arise.

The program is free to all citizens living in the Fallbrook, Bonsall, Rainbow and De Luz communities, and services are provided confidentially.
